DOT Physical Appointment Request
Currently DOT physicals are provided on Tuesday afternoons and Friday mornings by appointment only. Physicals cost $120 and include examination, urine analysis, medical certificate and DOT long form. Please bring a list of any current medications or supplements and be prepared to answer questions about your medical history including the name and dates of any prior surgeries or medical procedures.
